Bright Star Community Care description – This funding is to seek assistance from a Support Coordinator with Further qualifications/experience required to strengthen a participant’s ability to design and the build their supports with an emphasis on linking the broader systems of support across a complex service delivery environment. Coordination of Supports is to focus on supporting participants to direct their lives, not just their services. This may include resolving points of crisis, and developing resilience in the participant’s network.

OUR STAFF
With the level of education and skill you don’t just get Case Coordination you also can get Psychosocial Coaching and Recovery Coach. We have proper university education and high level experience. Not just a cert 4. There are no extra costs this quality staff – its what you should expect from you supports and health care providers.

We are certified by NDIS to work with both Children and Adults and we offer a specialised service for Mental Health Clients, Behaviour Support Practitioner for children and adults and we do assessments for Diagnosis.

ACTIVITIES WE ARE DOING BEHIND THE SCENE
- Assessing your needs and goals. We also considered family needs too and we are happy to work with hospitals, service providers, Justice systems, schools and Day Care if you require!
- We develop and regularly updates your Care Plan so we have Data to evaluate outrcomes and ensure your meeting your goals.
- We updates your goals as they change with what is happening in your day to day life.
- You can work with your own Support Workers or we can link you with Community resources, service providers or search for services to meet you needs.
- We will have regular team meetings with all your support providers and clinicians so we are up to date with where everyone is at and working for you to achieve your goals and adapting goals to need your needs as your situation changes.
- We prepare your NDIS Review Audits early on so all clinician’s involved in you our care have plenty of time to have their documentation ready to support your review with NDIS.
